Job Title: Social Worker (CSW / LSW / School Social Worker) Location: New JerseyJob Type: Full-Time Setting: Adult Day Care FacilityClient: ATC Healthcare Services About the Role ATC Healthcare Services is seeking a qualified and compassionate Social Worker holding a CSW, LSW, or School Social Worker certification to provide support services in an Adult Day Care setting. The Social Worker will play a key role in enhancing participants’ emotional well-being, coordinating care, and supporting families while ensuring compliance with state regulations. Key Responsibilities * Conduct psychosocial assessments and develop individualized service plans for adult day care participants. * Provide counseling, emotional support, and crisis intervention as needed. * Coordinate with healthcare providers, administrators, caregivers, and community resources to ensure continuity of care. * Assist participants and families with social services referrals, benefits, and community programs. * Maintain accurate and timely documentation and case records in compliance with state and facility requirements. * Participate in interdisciplinary team meetings and care planning. * Advocate for participants’ rights, dignity, and quality of life. * Ensure compliance with HIPAA and all applicable regulations. Qualifications * Current New Jersey certification/license as CSW, LSW, or School Social Worker (required). * Minimum 3 year of experience in social work, healthcare, senior care, or community-based services preferred. * Strong interpersonal, communication, and organizational skills. * Ability to work collaboratively with multidisciplinary teams. * Experience working with older adults or special needs populations is a plus.
